Does Ohio Medicaid cover braces?
Children and teens only. Ohio Medicaid covers comprehensive orthodontic treatment for patients younger than 21 when the case is medically necessary under the state's scoring form (22 points or an automatic qualifier, form ODM 03630, from January 1, 2026), with one prior authorization covering up to eight quarters. There is no adult orthodontic benefit.

Verify a Ohio license
An NPI record is not proof of an active license. CMS says so itself. Every profile on this site links eLicense Ohio's license verification (Ohio State Dental Board), and you can verify any Ohio dental license there in about a minute. If what you find disagrees with a listing here, tell us.
How this list works
Everything on this page comes from one source: the federal NPI register (NPPES), read on October 1, 2026. It lists every provider in Ohio whose register address matches, alphabetically. No one can pay to be added, removed or reordered, and there are no ratings or reviews here, because we have not called or visited these practices.
Three caveats the register itself requires: an NPI number is not proof of an active license (verify one at the Ohio license lookup), addresses are self-reported and can be stale, and a listing says nothing about quality. If you are listed here and something is wrong, tell us and we will correct or remove the entry against the register.
